Pharmacovigilance requirements in Liberia

In Liberia, Pharmacovigilance is regulated by the Liberia Medicines and Health Products Regulatory Authority (LMHRA). The system is basic and developing, functioning under the Medicine and Health Products Regulatory Act 2010. It operates with limited enforcement capacity, relying heavily on WHO guidelines and regional support from Africa CDC.

1. Clinical Safety (Clinical Trials)

Clinical trials are rare in Liberia but are strictly regulated when they occur, often drawing on lessons from public health emergencies like Ebola.

  • Approvals: Trials require authorization from the LMHRA and ethics review boards.

  • Expedited Reporting: Sponsors must report Suspected Unexpected Serious Adverse Reactions (SUSARs). While not rigidly codified in national statutes, the expectation aligns with international (ICH E2A) norms:

    • Fatal or Life-Threatening: Within 7 calendar days.

    • Other Serious Unexpected: Within 15 calendar days.

  • Periodic Reporting: Development Safety Update Reports (DSURs) are typically required annually or upon request (ICH E2F format).

  • Submission: Reports are submitted directly to the LMHRA via email or standard forms; there is no dedicated electronic trial portal.

2. Post-Marketing Pharmacovigilance

Post-approval surveillance is minimalistic, prioritizing national coordination over complex industry obligations.

  • Governance: The National Pharmacovigilance Centre (NPC) is situated within the LMHRA. It contributes to VigiBase via VigiFlow, though reporting volumes are historically low.

  • Personnel & Infrastructure:

    • No Local QPPV: There is no explicit requirement for a local Qualified Person for Pharmacovigilance residing in Liberia.

    • No PSMF Mandate: Marketing Authorization Holders (MAHs) are not required to register or maintain a Pharmacovigilance System Master File in the country.

  • Incident Reporting (ICSRs):

    • Reporting is spontaneous and voluntary, utilizing VigiFlow, email, or paper forms.

    • Timelines: Not rigidly codified. The expectation is "prompt" reporting for serious or unexpected events (generally interpreted as 15 days).

  • Risk & Periodic Reporting:

    • PSURs: Not routinely mandatory. Safety updates may be requested during registration renewal or variations.

    • Risk Management Plans (RMPs): Not a standard requirement; risk assessment is reactive.
